o Security Bank - use payment slip, cash and check payments
o Philtrust Bank - use deposit slip, cash and check payments
o MLhuillier - use payment slip, cash payment only, with service fee
Write complete name , student number (if applicable) and campus in the slip and KEEP a validated copy for reference.

Refer to your latest SOA on hand, if applicable, to ensure correct payment details. Fill-out all the essential information. Encode the correct amount to be paid. When paying offline to your chosen payment outlet: Provide a valid and active email address because the reference number will be emailed to you for presentation to your chosen payment outlet. Bring the reference number and pay within 24 hours. Keep the receipt as proof of payment. 5. For the applicant in admission, paying application fee put "0000-00000" in Student Number field.
